ITEM(S) SUBMITTED FOR APPROVAL

1.   Approval of Budget Amendment #11-030 in the amount of $77,043.00 to record unanticipated revenue from State of Florida, Division of Emergency Management, 2010-11 Emergency Management Preparedness and Assistance (EMPA) Grant. This is a cost reimbursement grant whose funds were allocated by the State to the County in Agreement/Contract #10/11-21, which was aproved by the County 10/26/2010 and by the State 12/1/2010. The funds are to be used to support Emergency Management operations.

2.   Approval of Budget Amendment #11-031 and Resolution in the amount of $94,777.00 to record unanticipated revenue from the Florida Department of Law Enforcement (FDLE), Edward Byrne Memorial Justice Assistance Grant #2011-JAGC-CLAY-1-B2-140. The funds are to be used for overtime salaries, operating expenses and capital equipment for the Drug Task Force. Funding Source: 102-102-331220 (K. Zebraski)  
 
3.   Approval of Budget Amendment #11-032 and Resolution in the amount of $23,866.00 to record unanticipated revenue from the Bureau of Justice Assistance (BJA), State Criminal Alien Assistance Program (SCAAP) Grant #2010-AP-BX-0022. The funds will be used for overtime expenditures associated with the Clay County Jail.

4.   Approval of Budget Transfer #11-033 in the amount of $83,829.18 to allocate funds in Public Safety-Administration/Communications Division to cover unbudgeted costs associated with the Emergency Operations Center (EOC). 5.   Approval of Budget Transfer #11-034 in the amount of $166,000.00 to secure additional funding for Bottomridge Stormwater Project. This is necessary due to overruns in permitting fees and construction costs. This transfer is out of CIP contingency in the same fund.

6.   Approval of Voting Precinct Lease Agreement for Voting Precinct 108 and Early Vote Sites 308, 406, 409, 601, 700 and Orange Park Library. (R. O'Connell)  
 
7.   Approval of a Railroad Reimbursement Agreement, and the accompanying Resolution, with the Florida Department of Transportation (FDOT) for annual maintenance of Crossing Device at C.R. 315B/Russell Road. The County is currently responsible for reimbursement to CSX for a portion of the maintenance of similar crossing devices within the County (excluding Municipalities). 8.   Approval of the First Amendment to Participation Addendum between Pasco County and OfficeMax to extend the expiration date to September 30, 2011. This agreement allows Clay County to purchase office products at a lower price given to Pasco County under a Master Agreement. Funding Source: Various (B. Johns/K. Thomas)  
 
9.   Approval of an Agreement with Florida Medical Training Institute to allow students from the Institute to gain practical experience with Clay County Fire/Rescue in exchange for classroom time for Fire/Rescue employees. No funding is required. (B. Johns/L. Mock)  
 
10.   Approval of an Agreement with Loblolly Mitigation Bank in the amount of $12,800.00, for the reservation of wetland mitigation credits for Old Jennings Road Improvements Project. Approval is also requested for advance payment and as a sole source vendor.

11.   Approval of an Agreement with Sundew Mitigation Bank in the amount of $20,000.00, for the reservation of wetland mitigation credits for New Castle Drive Wetland Restoration and Flood Control Project. 12.   Approval to post notice of intent and to award Bid #10/11-2, County Road 209 North Cross Drain, to Northpointe Services, Inc. as the lowest responsive bid. Approval will be effective after the 72-hour bid protest period has expired, assuming no protests are received. Submittals are available for review in the Purchasing Department.

13.   Approval of an Interlocal Agreement with Bradford County to facilitate their payments to Clay County in the amount of $35.00 each for inspection services and plans review services to be provided by our Building Department. (B. Johns/D. Conner)  
 
14.   Approval of an Agreement with the Florida Department of Transportation (FDOT) for construction, maintenance, and joint use of Pond “A” for the Old Jennings Road Widening Project. No funding is anticipated as a result of this agreement and it is expected to save the County a considerable amount as the cost of building our own retention pond will be avoided. Staff requests authorization to execute the Quitclaim Deed (Exhibit A) at the appropriate time. (B.
