1.   Approval of Budget Amendment #10-131 and Resolution in the amount of $20,000.00 to record unanticipated revenue from the U.S. Marshall Service. The funds are to be used by the Clay County Sheriff's Office to reimburse overtime expenses which have occurred from abducted/murdered child investigation.

4.   Approval of Budget Amendment #10-142 and Resolution in the amount of $3,638,523.00 to record unanticipated revenue from the Florida Department Community Affairs (DCA) to be used by the SHIP Program to purchase, rehabilitate, and resell or lease previously foreclosed properties currently owned by the foreclosing entities Real Estate Offices (REO). This is a result of Agreement/Contract #08/09-148.

5.   Approval of Agreement with Jeff’s Excavating in the amount of $43,463.00 for Constitution Drive Cross Drain Addition. This contract is the result of the awarding of Bid #09/10-12.

6.   Approval of Agreement with ADPI-Intermedix for services in connection with the collection of fees from EMS patients and their insurance carriers. This contract is the result of RFP #09/10-7.  ADPI will be paid a percentage based on their collections.

7.   Approval of Grant of Easement with the Clay County Utility Authority (CCUA) for Parcel #37-05-24-006795-001-00 to accommodate proposed water and wastewater utility extensions along SR 21 in Middleburg. Approval will result in $10.00 revenue received from CCUA.

8.   Approval to utilize Martin Environmental Solutions Inc. as a sole source provider for Microbial Remediation serives for the Orange Park Public Library. The estimated cost for all remediation services for this project is $35,000.00. Further sole source justification provided within.

9.   Approval of revised Sole Source language (previously approved at 1/13/09 BCC meeting) to include Dell as a sole source provider for future purchase of Dell hardware warranty extensions.

10.   Approval for David’s Permanent Coatings, Inc., as a sole source provider, to spray DOW HPAQ polyurea formula coating on the third floor shower area and mop room in the Clay County Jail, at a cost of $38,961.00. This coating will provide a water tight barrier to the area. Sole source justification provided in the attached memo.

11.   Approval of Economic Development Grants (EDG) for Project Bright and Project Well. The local EDG is performance based with upfront performance measures to be met and maintained by each company in order to be eligible to receive the full amount of the grants.
