ITEM(S) SUBMITTED FOR APPROVAL

1.   Approval of Resolution Authorizing Participation in a Regional Planning Assistance Program for FY 2011/2012 in the amount of $75,704.04. This request is contingent upon FY 2011/2012 budget approval. Funding Source: 001-0100-554000 (S. Kopelousos)  
 
2.   Approval of Second Renewal to Bid #06/07-48, Uniform Rental and Service to G & K Services. This renewal, which provides uniform rental and service, effectively extends the bid with no increase in cost for a one year period and also provides a 5% discount on all catalog products. Funding Sources: Various-552160 (Uniforms) (K. 3.   Approval of renewal of the County's stop/loss reinsurance coverage with Blue Cross Blue Shield per the attached quotes and recommendations. Funding Source: 110-8888-523001 (D. Moore)  
 
4.   Approval of a one-year contract extension for Administration Agreement #10/11-57 with Blue Cross Blue Shield which will extend the termination date to December 31, 2014. This will result in the deferral of the price increase (from $37.00 to $38.00 per participant, per month) for one year. Blue Cross/Blue Shield has agreed to a onetime payment back to the County of $100,000.00. Funding Source:110-8888-523001 (D. Moore/B. Johns)  
 
5.   Approval of a Lease Agreement with the Supervisor of Elections for the use of various County-owned sites as Voting Precincts during the 2012 elections. Locations are listed in the Lease. (S. Kopelousos/B. Johns)
 
 
6.   Approval of two new commercial waste collection service franchisees: Arwood, Inc. and Roof Consultants & Services, Inc. There are currently thirteen (13) franchise commercial waste haulers operating within the County and by Ordinance Clay County is permitted up to fifteen (15). There were seven (7) applications for the two franchises. The evaluation forms are attached. (A. Altman/B. Johns)  
 
7.   Approval of insurance renewals as follows:
  1. County’s property and casualty, general liability and public officials liability, per the attached recommendations, effective 10/1/2011. The lowest prices found results in an increase of $22,491.00 (3.5%). Funding Source: 001-0105-545000
  2. County’s workers compensation excess insurance coverage, per the attached recommendations, effective 10/1/2011, resulting in no increase in premium. Funding Source: 001-0105-524000
  3. County’s additional covered member coverage, per the attached recommendations with the Florida Sheriff’s Self Insurance Program, in the amount of $25,000.00, effective 10/1/2011. Funding Source: 001-0105-545000 (D. Moore)
 
 
8.   Approval of Second Amendment to Adult Drug Court (10/11-207) and Dependency Drug Court (10/11-208) funding agreements for FY 10/11. Clay Behavioral Health is requesting reallocation of the funding to increase some items and decrease other items to accommodate expenditures for the remainder of this fiscal year. The total amount will not change for either of the agreements. Funding Source: 134-1067-531000 (M. Scruby/ B. Johns)  
 
9.   Approval of Memorandum of Understanding with St. Johns River State College to allow students from the college to participate in practical learning experiences with Clay County Fire Rescue in exchange for classroom time for Fire/Rescue employees. No funding is required. (D. Motes/B. Johns)  
 
10.   Approval of annual funding agreement for FY 11/12 with the State of Florida Department of Health for the operation of the Clay County Health Department in the amount of $851,697.00. Funding Source: 001-0108-549100 (S. Kopelousos/B. Johns)  
 
11.   Approval for the Clay County Sheriff's Office (CCSO) to accept the 2012 Edward Byrne Memorial Justice Assistance Grant (JAG) award in the amount of $75,438.00 from the Florida Department of Law Enforcement (FDLE), Office of Criminal Justice Grants. The funds will be used for the Clay County Drug Task Force. (S. Kraus)  
 
12.   Approval of Leases (pending legal review) with the following athletic associations:
  1. Clay County Adult Softball
  2. Clay County Soccer Club
  3. Green Cove Springs Athletic Association
  4. Keystone Recreation Association
  5. Middleburg Athletic Association
  6. Tanglewood Athletic Association
The athletic associations pay the County $1.00 per year for the use of the facilities.
