12. | Resolution Authorizing the Clay County Development Authority to issue industrial development revenue bonds for the Haven Hospice project | ||||||
13. | Approval of the following items placed on the 10/17/11 Budget/Finance Committee agenda:
A.) Budget Amendment #12-006 and Resolution in the amount of $14,501.00 to record the receipt of unanticipated revenue from the State of Florida, Department of Health, Division of Emergency Medical Operations for the EMS Grant - C0010. The grant is to be used for improving and expanding pre-hospital emergency medical services. Funding Source: 001-001-334203 B.) Budget Transfer Nos. 12-009, 12-010 and 12-011 in the total amount of $15,224.02 to allocate funds in the 1) Clerk of Court, 2) Teen Court - Circuit Juvenile, and 3) Clerk of Court - Technology Divisions to correct budget to actual budget submission. The transfer places funds back into the appropriate Reserve for Contingency accounts. Funding Sources: 001-0701-516000, 125-1064-516000, 125-1064-549900 & 135-1066-516000 C.) Request by Clay County Sheriff’s Office (CCSO) for approval of Letter of Agreement, authorizing First Coast Crime Stoppers, Inc. as an agent to apply for Florida Attorney General grant funding on behalf of Clay County (see attachments). D.) FY 11/12 Funding Agreement with the Police Activities League (PAL) in the amount of $15,000.00 to provide youth activities program. Funding Source: 001-2505-582600 E.) An Agreement with the City of Jacksonville for continued funding of the emergency notification system (CodeRED). The items covered in the contract were purchased by the City of Jacksonville with federal grant funds from the 2009 Urban Area Security Initiative Program. There will be no charge to Clay County. F.) A Subordination of Utility Agreement for Henley Road with the Clay County Utility Authority. This Agreement is needed to subordinate utility interests of easement along the right-of-way (R/W) of Henley Road due to the acquisition of additional R/W over these easements. G.) Equitable Sharing Agreement and Certification in the amount of $1,235.43 plus $3.25 interest (total of $1,238.68) with the Clay County Sheriff's Office (CCSO). This document signifies equitable sharing of seized assets. |
