2. | Approval for the Clay County Historical Commission to apply for the Florida Department of State, Division of Historical Resources, Bureau of Historic Preservation Grant in the amount of $20,000.00. The grant funds are to be used for the stabilization of the Budington House in Middleburg. This is a 50/50 grant program which requires $20,000.00 match from the Historical Society; $15,000.00 in-kind labor and $5,000.00 County funds. 3. | Approval to modify bid award of a certain item for Bid #09/10-1, EMS Medical Supplies, due to vendor no longer being able to supply the item at the bid price, and award item 8.4% Sodium Bicarb Adult to the next low bidder Henry Schein. Funding Source: 001-2101-552000 (K. Thomas) |

4. | Approval to modify bid award for Bid No. 09/10-22, Reconditioning and Remounting of Rescue Chassis, to allow for a new year chassis (2012) with a price increase of $1,500.00. The bid is a one year bid with the intent to be used as needed over the one year period, however the bid currently does not allow for cost adjustments for newer year models. Staff recommends modifying the bid award to allow for the price increase for a 2012 chassis, rather than a 2011, and allow staff to negotiate trade in values when necessary. The Board has the option to approve the evaluation committee’s recommendation or should presentations be desired, it is requested that a short list of a minimum of the top three firms be invited to present to the full Board. 10. | Approval of staff's rankings for RFQ No. 10/11-3, Contractor Prequalification for Construction of the Green Cove Springs Senior Center. Staff’s evaluations of the submittals are attached hereto for consideration. The purpose of this RFQ is to qualify a pool of contractors to later bid on the Construction of the Senior Center Addition. The bid stated that the County reserved the right to qualify more or less than five contractors. Due to the tied scores, staff recommends qualifying the top eight to ten contractors.
